A website enquiry reaches the office so it can reply about the topic you named. There is no automatic marketing subscription.
The fields collected are name, phone, a general category, a preferred channel, and optionally email, a preferred time and a short message. Do not send a medical file, an ID number, court documents or third-party details through this form.
The form does not create attorney-client privilege. Ordinary email is not a suitable channel for medical records. If an external delivery channel is configured it is used first; otherwise the enquiry is stored in the server inbox so the office can reply. Names, phone numbers and messages are not written to logs.
If analytics is later enabled, it will not include names, phone numbers, messages or medical content.
